1.       Email to the ff. details:

a. Your Full Name, Contact Number, Email Address, Cignal Account Number

b. Your Referral’s

         i. Full Name, Complete Address, Contact Numbers, Email Address

        ii. Scanned copy of Cignal Subscriber Application Form, Proof of Billing, and valid ID.

2. Cignal Subscriber Application Form (SAF) can be downloaded here. You may also get a copy of Cignal SAF at any Cignal Partner. Complete list of Cignal Partner can be accessed at this link.

3. Indicate in email subject line “MGM” and your Cignal Account Name and Number. (e.g. MGM – Juan dela Cruz 1234567)

4. A sales representative will contact your referral to confirm his/her subscription details and account information.

5. Once confirmed, installation at referral’s address will be scheduled.

6. Referral’s account will be activated after successful installation.

7. Postpaid accounts that are installed until September 30, 2012 are entitled to get free 3 months only.

8. Bill rebate will automatically be given in the next applicable billing statement once referral’s account has been successfully installed and activated.

9. Cignal subscriber must be active in order to receive bill rebate.

10. For disconnected accounts, any outstanding balance should be settled before bill rebate will be given. A reconnection fee of Php 100 will be billed.

11. This promo is open to residential Cignal postpaid subscribers only.

12. This promo is NOT open to both prepaid Cignal subscribers and Cignal institutional accounts.

13. Cignal subscriber is not allowed to refer regular and probationary employees of MediaQuest, MediaScape, TV5, Media5 and NBC.

14. Promo runs until September 30, 2012. Per DTI-NCR Permit No. 2571, Series of 2012.
